Starting on Monday 5th December, we will be having our Always Safety First Safety week sharing and celebrating your contribution to safety and improvement across the Trust with a focus on Safety Huddles and Safety Pauses.
Safety huddles happen across our clinical areas and are a brief, focused, multidisciplinary and structured exchange of information which are held at a predictable time and place about potential or existing safety risks to proactively addresses or mitigate risk. They are informed by visual feedback data which is provided through the Trust safety surveillance dashboard and provide the opportunity to celebrate success in reducing harm. A safety pause is slightly different and can be enacted at any point throughout a shift by any member of staff who feels that there is a need to re-focus on safety, their structure will follow that of the pre-planned safety huddle that already happens on the ward.

During this week we encourage you to share contributions and successes to safety and improvement using the hashtag #LTHTRSafetyWeek so that we can all celebrate collectively, sharing our personal pledges towards patient safety. In addition, some of our Executive colleagues will be visiting wards during the week to observe some safety huddles.
